For your convenience, now you can download your New Patient paperwork here. We're also happy to fax it to you, if you prefer. Or you can wait until your first appointment and fill it in when you're here but you'll need to leave yourself fifteen to twenty minutes extra.
And please remember to contact your insurance company before you come in to verify what your policy covers for chiropractic care. We are happy to help you navigate your policy and provide support if you have questions regarding your insurance. However, it is important that you remember thatyour insurance is a contract between you (the patient) and the insurance company.
New Patient Paperwork:
On your first visit, expect to spend a few minutes filling out information forms. To help save you time and make your visit with us a little faster, we'll be happy to mail or fax the forms to you ahead of time (if time permits). Also, expect to provide some information regarding your general health. If you have insurance, you will be asked to provide further information about your coverage, including a copy of your insurance card.
Dr. Mark will review this information with you and will discuss your pain, discomfort and any issues relevent to your care. Following an examination, you can expect to receive a manipulation, if needed.
After your first visit, you can expect your subsequent visits to take about twenty minutes. Every patient is different and the frequency of your return visits can vary greatly. But keep in mind that ultimately you, the patient are the healer. We believe that YOU are in control of your care! We will recommend a course of treatment for you but it's up to you to decide what to do and what not to do.
Chiropractors do not give shots, prescribe medication, or do surgery. If Dr. Mark feels that you need X-rays, you will be scheduled for x-rays through a local facility.
